Rydon Construction has been fined £500,000 with a sub-contractor also fined £360,000 following an accident that resulted in the fatality of an employee falling from height.
The employee was crossing over a temporary covering which was in place over a shaft, when it gave way, resulting in a fall of approximately 14 metres.
The Health and Safety Executive found Rydon Construction failed to plan, manage and monitor construction work involving the temporary platform to ensure it was safe.
The company had also failed to identify the lack of design drawings for the platform or carry out suitable checks.
